Parent/Family Transportation Agreement
This form needs to be returned to Tucker Transportation by August 18th to be eligible for
transportation. After August 18th, there may be a waiting period before student(s) are eligible to ride.
By signing below, I agree to the following terms and conditions:
Must reside on established route to be eligible to ride.
Family units sit together.
Seats will be assigned to all students.
Absolutely NO FOOD OR DRINKS on the bus.

Please note that the School District is not responsible for transporting students unless they live 3
miles or more from the school.
We will also be making the following changes to bus 94 - this is being done due to the high number of
students on this route.
In order for your student to ride bus 94:
You must be a Preschool/Early K student that does not reside on the other routes.
or
You must have a need for transportation written into your IEP. We noticed that there were
many students riding this bus last school year that were ineligible. Therefore, only students
with this service in their IEP's will be eligible for transportation.
